The California Healthy Kids Survey (CHKS) will be administered to students in grades 5, 7, 9 and 11 this winter. Parents who wish to preview the survey questions prior to their child’s participation can do so using the links below.
Parents of 5th-grade students must opt their child in to participate.
Parents of 7th, 9th and 11th-grade students can opt their child out if they choose.
